New Patients
Our office requires 2 Progress Notes, and any pertinent lab/x-ray/imaging results. These documents must be received prior to scheduling an appointment with our office. Please note that we do not accept Medicaid or Dual Complete. Please call our offices with questions regarding which insurance plans we accept.
Late Policy Notice
Your check-in time is 15 minutes prior to your scheduled appointment if you arrive on or after your appointment time, you may be asked to reschedule.
